Online Check-In Available. When you call us for an appointment we will send you an appointment confirmation text. At the bottom of the text, we request you send us back a text with your driver's license number, first name, middle name, last name, date of birth, mobile phone number, and email address. You will then receive a text and email from FormFox to complete the online questionnaire. You will be able to complete the questionnaire by smartphone or computer.
