If you intend to visit your doctor before your examination and are unsure of your medical health condition please ask them for a copy of your office visit note to bring to your examination e.g. Blood pressure readings if they are normal or a history of controlled diabetes.
If you have had a heart attack, heart surgery or other major heart procedure you must receive clearance from your attending cardiologist with- in the past two years. Non-Drug testing of urine is required and will be performed during the exam. If your employer requires drug or alcohol testing these services are available at an additional fee.
The DOT/CDL physical is intended to aid in the safety of yourself and other drivers sharing the road.
The FMCSA division of the DOT requires all commercial drivers licensed in the USA to get regular examination from a member of the National Registry of Certified Medical Examiners (NRCME). Dr. Arnoldo Daniel Gutiérrez is a member in good standing with the NRCME.
What to Expect During Your DOT Visit
Step One: Medical History
To provide an idea of your general health and family background, you'll be asked to complete a brief medical history detailing past illness, diseases that run in your family, your overall health and you will be asked to list any drugs you currently take.
Step Two: The Exam
Once you've provided your medical history we will review the information you've provided, go over any medical conditions you may have, and perform a basic urinalysis test. As part of your exam, we will also: measure your height, your weight, take your blood pressure, check your vision and hearing. We will also make a note of whether or not you wear glasses or contact lenses to correct your vision and whether or not you use a hearing aide. Dr. Gutiérrez will test your range of motion and check your breathing.
Dr. Gutiérrez will also perform the following tests:
- Eye, throat, mouth, and ear examination
- Aural heart and lung examination
- Check for deformities of the spine
- Neurological examination
- Check for abdominal abnormalities
- Hernia check
After these physical examinations are complete, your care provider will discuss any hazards that may be created by your personal health history and any current medical conditions you may have. Any present hazards will be documented and used to determine whether or not you meet the standards required to receive an MEC.
Step Three: Paperwork
The final step in the process is the paperwork. If your care provider determines that you do meet the qualifications, you will be given an MEC and will not have to take another DOT exam for two years. After two years, you will be required to take the exam again in order to keep your CDL. Some medical examiners will give an examination that is good for three months, six months, or one year. Those who do not meet the standards cannot be given a medical certificate for their CDL licensing requirements. Before you leave the office, you'll get a copy of the exam to take with you when you get your CDL, and a second copy to give to your company. We'll also keep a scanned copy of the exam in your file at the office. And after all the paperwork has been processed, you'll get your very own DOT medical card to keep with you at all times when you're driving.

- Medical Records/Prescriptions/Medication Bottles
- Clearance to commercially drive letter from Cardiologist if you have had a heart attack or heart bypass surgery. Bring documentation of a properly functioning pacemaker if you have one.
- Copies of previous blood pressure reading or statements from your Primary Care Doctor if you have issues with you blood pressure e.g. white coat syndrome.
- Clearance letter to drive if you take a scheduled narcotic, or amphetamine for any reason e.g. ADHD, anxiety, depression or weight loss etc.
Urine Test is Mandatory: This is not a drug test. However, FMCSA applications do inquire about illegal drug use.
Get a good night sleep, avoid smoking and coffee drinking prior to the examination.
Avoid fatty and sugary foods the night before and morning of your exam.
Eat Healthy and Don't Smoke prior to the examination
Eat a light healthy meal the night before and a healthy breakfast. Smoking raises blood pressure. Your blood pressure must be below 140/90 when measured at time of examination. Prior to your exam you should avoid coffee, tea, sugary beverages, sodas, energy drinks, sport drinks and smoking.

CDL Medical Card High Blood Pressure
CDL/DOT Physical Exam – High Blood Pressure Requirement
“A person is physically qualified to drive a commercial motor vehicle if that person — Has no current clinical diagnosis of high blood pressure likely to interfere with his/her ability to operate a commercial motor vehicle safely.”
If a driver has hypertension and/or is being medicated for hypertension, he or she should be recertified more frequently.
An individual diagnosed with Stage 1 hypertension (BP is 140/90–159/99) may be certified for one year. At recertification, an individual with a BP equal to or less than 140/90 may be certified for one year; however, if his or her BP is greater than 140/90 but less than 160/100, a one-time certificate for 3 months can be issued.
An individual diagnosed with Stage 2 (BP is 160/100-179/109) should be treated and a one-time certificate for 3-month certification can be issued. Once the driver has reduced his or her BP to equal to or less than 140/90, he or she may be recertified annually thereafter.
An individual diagnosed with Stage 3 hypertension (BP equal to or greater than 180/110) should not be certified until his or her BP is reduced to 140/90 or less, and may be recertified every 6 months.
CDL Medical Card Diabetes
CDL/DOT Physical Exam – Diabetes Mellitus
The complications of diabetes mellitus can lead to medical conditions severe enough to be disqualifying, such as neuropathy, retinopathy, and nephropathy. Accelerated atherosclerosis is a major complication of diabetes mellitus involving the coronary, cerebral, and peripheral vessels. Individuals with diabetes mellitus are at increased risk for coronary heart disease and have a higher incidence of painless myocardial infarction than individuals who do not have diabetes mellitus.
Preventing hypoglycemia is the most critical and challenging safety issue for any driver with diabetes mellitus. Hypoglycemia can occur in individuals with diabetes mellitus who both use and do not use insulin. Mild hypoglycemia causes rapid heart rate, sweating, weakness, and hunger. Severe hypoglycemia can cause symptoms that interfere with safe driving.
With Good Management A Driver With Diabetes Can Safely Operate A Commercial Motor Vehicle
The Center for Disease Control and Prevention (CDC) 2007 National Diabetes Fact Sheet reports the prevalence of diagnosed and undiagnosed diabetes mellitus in the United States, for all ages, as:
- Total: 23.6 million people, or 7.8% of the population, have diabetes.
- Diagnosed: 17.9 million people.
- Undiagnosed: 5.7 million people.
The most common form of diabetes mellitus is Type 2 (adult onset or non-insulin-dependent diabetes mellitus). Individuals with Type 2 diabetes mellitus:
- Can produce insulin and have intact blood glucose control counter-regulatory mechanisms.
- May preserve blood glucose control counter-regulatory mechanisms for many years with lifestyle changes and oral hypoglycemic medications.
- May, over time, have insulin production fail and require insulin replacement therapy.
While the detection and management of both hyperglycemia and hypoglycemia are important aspects of the overall medical management of a person with diabetes mellitus, the detection and management of hypoglycemia is more relevant to safety considerations, in the certification of the commercial motor vehicle (CMV) driver, with diabetes mellitus.
Blood Glucose Control
Some of the factors related to commercial driving that affect blood glucose control include:
- Fatigue.
- Lack of sleep.
- Poor diet.
- Missed meals.
- Emotional conditions.
- Stress.
- Concomitant illness.
These same factors may hasten the need for the driver with diabetes mellitus who does not use insulin to start insulin therapy. Poorly controlled diabetes mellitus can result in serious, life-threatening health consequences. However, with good management of the disease process, a driver with diabetes mellitus can safely operate a CMV.
CDL Medical Card Sleep Apnea
CDL/DOT Physical Exam – Sleep Apnea
Sleep disorders are quite common, but they can impact driving. As a result, the FMCSA has established guidelines regarding medical issues involving sleep. The good news is that sleep disorders will not necessarily instantly preclude you from passing the DOT physical.
Getting Certified with Sleep Apnea
You may be able to get certified even if you have sleep apnea. For example, if you use a CPAP machine and change your lifestyle to a healthier one to facilitate better and more restful sleep. The reason is that a CPAP machine can make a tremendous impact on sleep apnea. If you already have a CPAP machine make sure you print out your compliance report and bring it with you to the appointment.
If you snore and are obese, it might mean you are at risk of sleep apnea. If the examiner believes you may have sleep apnea, our Certified Medical Examiner and Clinical Coordinator will go over your options to learn more about your condition.
The commercial driver obviously spends more time driving than the average individual.
Driving is an activity that demands the driver be alert at all times. Symptoms of respiratory dysfunction or disease can be debilitating and can interfere with the ability to remain attentive to driving conditions and to perform heavy exertion. Even the slightest impairment in respiratory function under emergency conditions (when greater oxygen supply may be necessary for performance) can be detrimental to safe driving.
There are many primary and secondary respiratory conditions that interfere with oxygen exchange and may result in gradual or sudden incapacitation, for example:
- Asthma.
- Carcinoma.
- Chronic bronchitis.
- Emphysema.
- Obstructive sleep apnea.
- Tuberculosis. In addition, medications used to treat respiratory conditions, both prescription and those available without a prescription, may cause cognitive difficulties, compound the risk for excessive daytime sleepiness (EDS), or cause other forms of incapacitation.
CDL Medical Card Urinalysis
CDL/DOT Physical Exam – Urinalysis
We will record the test results of the required dipstick urinalysis (UA) in the Laboratory and Other Medical Test Findings section of the Medical Examination Report form. The dipstick urinalysis must measure specific gravity and test for protein, blood, and glucose in the urine. Positive test results may indicate that additional evaluation is needed.
Test for:
- Specific gravity.
- Protein (proteinuria).
- Blood (hematuria).
- Glucose (glycosuria).

CDL/DOT Physical Exam – Hearing Loss
To meet the Federal hearing standard, the driver must successfully complete one hearing test with one ear.
Regulations — driver must:
- First perceive a forced, whispered voice in one ear at not less than five feet, OR
- Not have an average hearing loss in one ear greater than 40 decibels (dB) at 500 hertz (Hz), 1,000 Hz, and 2,000 Hz.
A driver may use a hearing aid to meet the standard.

CDL/DOT Physical Exam – Vision Requirements
To meet the Federal vision standard, the driver must meet the qualification requirements for vision with both eyes.
Regulations — driver must have:
- Distant visual acuity of at least 20/40 (Snellen) in each eye, with or without corrective lenses.
- Distant binocular visual acuity of at least 20/40 (Snellen) in both eyes, with or without corrective lenses.
- Field of vision at least 70° in horizontal meridian in each eye.
- Ability to recognize and distinguish among the colors of traffic signals and devices showing the standard red, amber, and green.
